PSI Products For Sale

Patient Signature Logs are available for purchase through PSI. This form combines the log for third party prescriptions, counsel by pharmacist and acknowledgement of receipt of private practice notices.

Controlled Substance Inventory Lists for Michigan pharmacies are available for purchase through Pharmacy Services Inc. Michigan law requires an annual inventory of controlled substances to be completed not more than 30 days prior to May 1 and no later than 60 days after May 1.

Pseudoephedrine Logbooks to assist with compliance, Pharmacy Services Inc. has developed a pseudoephedrine logbook for retailers that meet federal, Michigan, and Ohio requirements.
